Transition Assessment is the ongoing process of gathering information related to an individual’s skills, interests, values, strengths and needs as they relate to life after high school. By understanding how to gather and use this information, families may assist the transition of their youth as well as begin their own parenting transitions.
-identify “quick & dirty” transition assessments
-identify good transition assessment & its implementation
-identify strengths, needs, interests and preferences
-assist in the development of the SOP (Summary of Performance) and
understand its use post high school
